Applying for Sponsorship support

Daiichi Sankyo reviews applications for Sponsorship on a quarterly basis. The table below presents the submission deadline dates for sponsorship review periods (by quarter):

  • Applications must be completed submitted online by the required date for each quarterly review period. Applications submitted after the required submission date will only be reviewed/considered during the next quarterly review period.
  • When making an online application, please be sure that all forms are accurately completed and that all required documents are provided electronically. Any missing or incorrect information may result in delays or rejection of applications.

Please note that sponsorship requests are not medically reviewed nor approved by the Office of Grants and Education (OG&E). All externally submitted sponsorship requests received by the OG&E are forwarded to appropriate personnel within Daiichi Sankyo, Inc., for processing. 

When applying for sponsorship support, patient and professional organizations and institutions should provide a full description of the event or program being sponsored and any tangible benefit opportunities (such as an exhibit or symposia space, or tickets or admission  to an event) being offered as part of the sponsorship.  Corporate recognition is not considered a tangible benefit.  Daiichi Sankyo will only accept tangible benefit opportunities which are consistent with Daiichi Sankyo policies and FDA regulations and approved labeling.

Additional sponsorship requirements:

Listed below are additional requirements for sponsorships:

  • Daiichi Sankyo will only provide sponsorship to the conference's sponsor; sponsorships will not be provided to individuals
  • Daiichi Sankyo will only sponsor programs and events that are held at locations where the gathering is primarily dedicated to promoting objective scientific and educational information, and the main incentive for bringing attendees together is to further their knowledge on the topic(s) being presented.
  • Daiichi Sankyo requires that a budget for the event or program be submitted with the request, outlining the estimated costs for the event and program
  • Daiichi Sankyo requires that the sponsorship support be used to off-set costs relating to the specific event or program being supported and may not be used for other purposes.
  • Acceptance of tangible benefits as part of the sponsorship:
    • If the tangible benefit includes exhibit or symposia space, Daiichi Sankyo will maintain control of all Daiichi Sankyo activities and materials
    • If the tangible benefit includes tickets or admission to an event, Daiichi Sankyo will only accept those number of tickets which can be used and will return any unused tickets

Application Steps

  1. Review the Requirements
  1. Complete application online; Attach required documentation.
  • Complete Funding Request Application for Sponsorship
  • IRS determination letter confirming the organization's 501(c)(3) tax status
  • IRS Form W-9 Tax Identification Number and Certification
  • IRS Form 990 (Return of Organization Exempt From Income Tax)For product donations, certification that the organization is licensed to receive, handle, ship, store and/or dispose of donated prescription drug product.
  1. Confirmation of Receipt
  • Daiichi Sankyo will send an email confirmation regarding the receipt of the application
  • If there are any questions on the application, a representative from Daiichi Sankyo may follow-up by email or phone
  1. Notification of Decision
  • Daiichi Sankyo will send an email notification within 10 days following the quarterly review date regarding the decision on whether or not to provide support
  • If there is a decision to support the application, instructions on how to proceed to a Letter of Agreement will be provided
